Referral Form

Online Referral Form

We have found that more patients follow through on scheduling appointments with a psychiatrist or psychotherapist when their provider sends us a referral form directly. Upon receiving a referral form, our staff will contact your patient to let them know we are working on finding a provider who accepts their insurance, has an available appointment, and is in the patient’s preferred city.

Fast

With one phone call, new patients will promptly be provided with an appointment to see one of our providers. We are able to schedule most appointments for non-urgent matters in 7-10 business days.

Telemedicine

Patients can connect with our providers from your office or from their homes using HIPAA-compliant video connections.

Easy Access

Our staff understands the challenges of mental health insurance and will work closely with your patient to verify their benefits, obtain authorizations for care and help them understand their mental health benefit.

All Ages

Nearly all of our locations have providers that are available to see child, adolescent, and adult patients.

Ongoing Communication

Community Psychiatry obtains a release form from the patient allowing our providers to communicate diagnosis, treatment plan, patient progress and outcomes back to the referring provider.

Unbiased Assistance

Patients, physicians or therapists can call our group to request assistance locating any of our providers who have open availability for new patients from the patient’s health plan list.

Affordable

We accept almost all commercial insurance, as well as Medi-Cal plans through Anthem Blue Cross and Beacon. If for any reason we do not accept your patient’s insurance, we will do our best to help find them a provider who does.

All Behavioral Health Problems

We treat patients of all diagnosis and specialize in medication management. Community Psychiatry collaborates with all psychotherapists in the community and will coordinate care for patients who need therapy in conjunction with their medication.
